Our In-Community Clinic brings care directly to you, whether in the community or at trusted locations like House of Mercy, Trillium Health Harm Reduction Services, and ROCovery. We create a space where you feel seen, heard, and respected, supporting individuals navigating substance use or those impacted by a loved one’s use.
This flexible program is designed for people who have had difficulty connecting with traditional clinic settings – facing challenges such as unstable housing, unmet basic needs, or involvement with multiple systems.
