At Delphi Rise, we are your one-stop source for re-entry support, offering a seamless network of services to help you thrive. Our Case Managers are here to connect you with the resources you need, whether it’s accessing nutritious food through our on-site pantry or getting immediate substance use evaluations and Peer support at our 24/7 Open Access Clinic. For those on the path to recovery, our on-site outpatient substance use clinic provides the care you need—with no waiting list—so you can start your journey right away.

Who is Eligible for Assistance?
Individuals formerly incarcerated in NY state prison and living in Monroe County are eligible.
If you’re unsure about eligibility, please reach out—we’re here to help.
Our Services Include:
Basic Needs and Financial Assistance
- Assistance with applying for Public Assistance, Medicaid, and SNAP
- Help with housing searches and applications
- First month’s rent and security deposit assistance
- Support with up to one month’s back rent to avoid eviction
- Transportation assistance and bus passes
Health and Wellness Support
- Cognitive Behavioral Intervention groups to develop positive coping strategies
- On-site substance use disorder evaluations, treatment, and referrals—including immediate access to our outpatient clinic
- Immediate connection to our Health Homes Care Management services
- Referrals to mental health treatment and interpersonal violence/anger management supports
Employment and Personal Development
- Work Readiness Programs to prepare for the workforce
- Help with obtaining identification to access work and other essential services
- Access to clothing and hygiene items to support daily living
Employment Specialist Support
Our Employment Specialist can assist with:
- Crafting professional resumes and job applications
- Training and certification opportunities for enhanced skills
- Mock interviews to boost confidence and preparedness
- Connecting individuals with re-entry-friendly employers, increasing employment opportunities
Re-Entry Peer Support
Our dedicated Re-Entry Peer—an individual with lived experience—can offer:
- Transportation to and from critical appointments
- Direct connection to the Delphi Rise Food Pantry for food, clothing, and hygiene needs
- Guidance and linkage to other necessary services and community resources
On-Site Food Pantry Program
- Supplies of food, clothing, and hygiene items to support stability
- Peer support to navigate health-related social needs
