At Delphi Rise, we believe no one should have to navigate the healthcare system alone. That’s why we’re here to connect you with the services that matter most—whether it’s medical care, mental health support, housing assistance, food resources, or transportation.
How It Works
1. Find Out If You’re Eligible
If you are a Medicaid recipient and struggle with two more chronic health conditions—such as asthma, diabetes, a mental health condition, or a substance use disorder—you may qualify for Health Homes Care Management services. If you are diagnosed with Serious Mental Illness, HIV/AIDS or Sickle Cell Disease, you automatically qualify.
2. Complete a Referral
Make a Referral – HHUNY You can select Delphi Rise as your preferred provider.
3. Get Connected with a Care Manager
Once enrolled, you’ll be paired with a dedicated Delphi Rise Care Manager—your personal advocate who listens, understands your needs, and helps create a plan based on your goals. They will make sure you get the care you need and deserve.
4. Access the Right Services
Your Care Manager will help you connect to:
- Doctors and specialists for medical care
- Substance Use Disorder treatment at our on-site outpatient clinic
- Our 24/7 Open Access Walk-In Clinic, providing support when you need it, no appointment required
- Housing supports, SNAP, childcare and other critical services
- Our on-site food pantry, stocked with healthy food for you and your family. We also offer cooking classes, trips to the public market and nutrition education
- Transportation assistance to medical appointments
- Other essential services to help you stay healthy and independent, and enjoy life
